2025 Feeder Registration

In consideration of being allowed to participate in the programs, related activities and events of Northbrook Baseball, Inc., participant and his/her parent or guardian acknowledges, appreciates, and agrees that:

1. Participation includes possible injury from participation in the programs, related activities and events of Northbrook Baseball, Inc. and possible exposure to illness (including but not limited to COVID-19);

2. It is impossible to fully eliminate the risk that I could be injured while participating in the programs, related activities and events of Northbrook Baseball, Inc.;

3. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES (as defined below) or others, and assume full responsibility for my participation in the programs, related activities and events of Northbrook Baseball, Inc.;

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Northbrook Baseball, Inc. and their officers, officials, umpires, coaches, assistant coaches, volunteers, other participants, sponsors, and owners and lessors of premises used to conduct any participating event (collectively the "RELEASEES"), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, INJURY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS ASSUMPTION OF RISK AND WAIVER OF LIABILITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY ACCEPTING IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

By Submitting this Registration/Electronic Signature, I certify that I, as parent/legal guardian with legal responsibility for this participant, do consent and agree to his/her waiver of liability as provided above of all the RELEASEES, and, for myself, my heirs, assigns and next of kin, I release and agree to indemnify the RELEASEES from any and all liabilities incident to my minor child's involvement or participation in the programs, related activities and events.
